彩色多普勒超声对胎儿单脐动脉的诊断及临床意义

Ultrasound diagnosis and clinical value of single umbilical artery before delivery

目的探讨彩色多普勒超声显像(CDFI)对胎儿单脐动脉(SUA)的诊断及临床价值。方法选择进行产前超声检查的10574例孕妇,使用CDFI观察胎儿膀胱旁动脉数目。对SUA胎儿合并的其它结构畸形及胎儿预后进行分析。结果共检出SUA胎儿51例,检出率0.49%,单纯性SUA 38例;合并其它畸形13例(其中泌尿系畸形3例,复杂心血管畸形5例,中枢神经系统畸形2例,消化道畸形2例,面部畸形1例)。继续妊娠的39例胎儿中,有5例在孕程和产程中出现各种异常,而34例胎儿妊娠进程顺利,随访无明显异常发现。结论彩色多普勒超声检查能够准确诊断胎儿单脐动脉,超声检查发现胎儿单脐动脉时需多角度动态观察以明确是否合并其他器官的发育畸形,对于继续妊娠者需密切注意胎儿发育情况和产程。

Objective To explore the diagnosis and clinical value of the color Doppler flow imaging（CDFI） for the single umbilical artery（SUA）.Methods 10,574 cases of pregnant women were observed the number of arteries next to fetal bladder.SUA fetus combined with other malformations and fetal prognosis was analyzed.Results Among all fetuses,there were 51 fetuses of SUA and the detection rate of SUA was 0.49%.38 of the 51 cases had malformations of single umbilical artery alone and the rest 13case were accompanied by many other organic malformations（3 cases of urinary tract malformation,complex cardiovascular malformation in 5 cases,2 cases of central nervous system abnormalities,gastrointestinal malformations 2 cases,facial deformity 1 case）.In the cases going on with their pregnancy,five cases had disorders during the pregnancy course or the labor,the other 34 cases were fine and there were no abnormalities in the following-up.Conclusion CDFI can diagnose SUA correctly.When we find afetus with SUA,we should analyze it from different aspect to except other organic malformations abnormality.
