中药结合髓芯减压术治疗早期股骨头坏死疗效观察

Curative Effect Observation of Chinese Drug Combined with Core Decompression on Caput Femoris Necrosis

目的：以早期股骨头坏死患者为研究对象,通过临床观察探讨中药结合髓芯减压术治疗早期股骨头坏死的疗效,为股骨头坏死的保髋治疗提供临床依据。方法：临床观察将42例ARCO I期、Ⅱ期的早期股骨头坏死患者,随机分为对照组20例26髋,治疗组22例28髋。对照组单纯采用中药治疗,治疗组采用中药结合髓芯减压术治疗。每3个月为1个疗程,两个疗程后进行疗效判定。结果：治疗组优7髋,良16髋,可5髋,差0髋,总有效率100.0%（95%C I=87.7%~100.0%）;对照组优1髋,良11髋,可12髋,差2髋,总有效率92.3%（95%C I=74.9%~99.1%）;两组综合疗效比较（u=2.760 1,P=0.007 7）,具有显著性差异。结论：中药结合髓芯减压术治疗早期股骨头坏死较单纯中药治疗能较好地减轻甚至消除患者病髋疼痛、改善病髋活动度和下肢生理功能,促进骨修复。

Objective： To observe the curative effect of Chinese drug combined with core decompression on caput femoris necrosis,and provide the clinical evidence for protecting coxa.Method： 42 cases with caput femoris necrosis in ARCO in stage I and stage II were divided into two groups randomly.20 cases（26 coxas） in control group were treated with Chinese drug only,and the other 22 ones（28 coxas） in treatment group were done with Chinese drug and core decompression.One course of treatment included 3 months.After two courses of treatment the curative effect was evaluated.Result： In treatment group 7 coxas was recovered,16 ones excellent,5 ones effective and no one ineffective.The total effective rate was 100%（95%CI=87.7%-100.0%）.In control group 1 coxa was recovered,11 ones excellent,12 ones effective and 2 ones ineffective.The total effective rate was 92.3%（95%CI=74.9%-99.1%）.There was significant difference between the two groups on the effective rate（u=2.760 1,P=0.007 7）.Conclusion： Chinese drug combined with core decompression can relieve or eliminate the pain in coax among caput femoris necrosis better than only Chinese drug,improve coax activity and the physiologic function of lower extremity,and promote bony frame to repair.
