间歇充气加压治疗预防高血压脑出血术后深静脉血栓的临床对照研究 被引量:5

Clinical comparison of intermittent pneumatic compression preventing deep venous thrombosis following hypertensive intracerebral hematoma operations

摘要 目的评估高血压脑出血术后病人应用间歇充气加压治疗预防深静脉血栓的临床疗效。方法将符合入选条件的65例高血压脑出血术后患者分为预防组和对照组。预防组33例,术后第1~14天皮下连续注射低分子肝素钙0.2ml/d,同时进行间歇充气加压治疗。对照组32例,仅行抗凝治疗。2组患者于术后1、4和14d分别行双下肢深静脉彩色超声多普勒检查,观察下肢深静脉血栓发生情况。结果预防组33例中仅1例(3.0%)发生下肢深静脉血栓形成。对照组32例中有8例发生下肢深静脉血栓(25.0%),其中6例(18.8%)发生在瘫痪侧,7例(21.9%)发生在腘静脉以下,2组下肢深静脉血栓形成发生率差异有统计学意义(P<0.05)。2组均无肺栓塞发生。结论高血压脑出血术后患者应用间歇充气加压治疗预防深静脉血栓形成是安全有效的,应大力推广。 Objective To evaluate the outcome of intermittent pneumatic compression in preventing deep venous thrombosis of patients following hypertensive intracerebral hematoma operations.Methods Sixty five patients were divided into two groups,the intermittent pneumatic compression group and the control group.33 cases in the intermittent pneumatic compression group,not only got Low-molecular-weight heparin 0.2 ml/L subcutaneous infusion from 1 to 14 days continuously after hypertensive intracerebral hematoma operations,but also treated with intermittent pneumatic compression.32 cases in the control group,just got Low-molecular-weight heparin subcutaneous infusion from 1 to 14 days continuously with the same dosing.All the patients were detected with color doppler flow imaging to investigate the incidence of deep venous thrombosis in 1,4,14 days after operations.Results Only 1 case of DVT(3.0%)was found in the intermittent pneumatic compression group.8 case of DVT(25.0%)were found in the control group,6 of them(18.8%)happened in the paralysis side,7 of them(21.9%)happened the lower part of the iliofemoral venous,there was statistical differences between the two groups(P0.05).Conclusion Using intermittent pneumatic compression to prevent deep venous thrombosis following hypertensive intracerebral hematoma operations is a safety and effectiveness method.
