摘要
目的探讨Ptfv1终末电势在诊断舒张性心力衰竭中的临床意义。方法随机选取128例的门诊就诊患者,根据临床症状、体征、超声多普勒检查分为无心力衰竭组、可疑舒张性心力衰竭组及舒张性并收缩性心力衰竭组。可疑舒张性心力衰竭组再以血浆B型钠尿肽(BNP)cut-off值62pg/ml为标准进一步分为两个亚组。Ptfv1≤-0.03mm.s为阳性。结果心力衰竭(包括舒张性心力衰竭、收缩性心力衰竭)组Ptfv1阳性率明显高于非心力衰竭组(P<0.05);可疑舒张性心力衰竭中BNP>62pg/ml组的Ptfv1阳性率明显高于BNP<62pg/ml组(P<0.05)。结论以Ptfv1≤-0.03mm.s为阳性标准,心电图Ptfv1是舒张性心力衰竭的一个重要的、有价值的辅助诊断指标。
Objective To evaluate the clinical value of Ptfv1 in the diagnosis of chronic diastolic heart failure.Methods To select clinical materials of the 128 out-patient clinic patients including the e1ectrocardiogram,UCG and plasma B-type natriuretic peptide（BNP） information.Those materials were divided into the following groups：the non-myocardial-failure group,suspecting of diastolic heart failure group,chronic diastolic heart failure group and chronic diastolic and systolic heart failure group.According BNP cut-off 62pg/ml suspecting of diastolic heart failure group were divided again into two group.When the Ptfv1 is lower than-0.03mm·s,the result is considered as the masculine.Results The Ptfv1 masculine of chronic heart failure group including diastolic and systolic heart failure patients appears higher than the one of non-myocardial-failure group（P〈0.05）.The Ptfv1 masculine ratio in exceed BNP 62pg/ml group appears higher than that of in less BNP 62pg/ml group（P〈0.05）.Conclusion The Ptfv1 masculine（Ptfv1-0.03mm·s） is thought as an important valuable criterion in the diagnosis on chronic diastolic heart failure.
