5种皮瓣在指中远节软组织缺损修复中的应用

Application of 5 kinds of flap in finger soft tissue defects

目的探讨指端组织缺损不同皮瓣修复方案及其临床疗效。方法回顾性分析采用不同皮瓣治疗94例患者130个指皮瓣:23例V-Y皮瓣、12例邻指皮瓣、63例指动脉逆行岛状皮瓣、17例邻指动脉逆行岛状皮瓣、15例腹部带蒂皮瓣的治疗效果。结果本组所有皮瓣全部成活。除15例腹部带蒂皮瓣的功能及外观差以外,其余各指恢复良好。结论 V-Y皮瓣修复指端损伤效果最好;邻指皮瓣、指动脉逆行岛状皮瓣和邻指动脉逆行岛状皮瓣修复效果较好;腹部带蒂皮瓣用于修复指脱套伤效果不够满意。

Objective To summarize the clinical efficacy of different flap program in the restore of fingertip tissue defects.Methods 94 Patients with 130 different parts of finger skin defects were treated with different flaps：23 cases V-Y advancement flap,12 cases cross-finger flap,63 cases reverse-flow finger pedicle skin flap,17 cases reverse-flow cross-finger pedicle skin flap and 15 cases abdomen pedicle flap.Results All these flaps survived and present satisfactory function and appearance except 15 cases of abdominal pedicle flap.Conclusion The clinical efficacy V-Y advancement flap is best cross-finger flap,reverse-flow finger pedicle skin flap and reverse-flow cross-finger pedicle skin flap are better.However,further improvement is still needed for abdominal pedicle flap in the treatment of finger degloving injury.
