奥曲肽治疗上消化道大出血疗效观察

Effect of Octreotide on Upper Gastrointestinal Bleeding

目的探讨奥曲肽治疗上消化道大出血的疗效,并与奥美拉唑的疗效作比较。方法我院2007年1月～2010年1月共收治80例上消化道出血的患者,随机分为治疗组和对照组各40例。治疗组采用奥曲肽(成都天台山制药有限公司)首剂0.1mg+生理盐水30mL静脉推注,继以0.2mg+5%葡萄糖注射液500mL中,静脉滴注维持,12h1次,持续2～3d;对照组采用奥美拉唑(江苏康缘药业有限公司)40mg+生理盐水150mL静脉滴注,每日2次。观察比较两组的临床疗效及止血效果。结果治疗组总有效率为90.00%(36/40),对照组总有效率为72.50%(29/40),两组总有效率比较,经χ2检验显示,差异有统计学意义(P<0.05)。治疗组的止血时间明显短于对照组,输血量也明显少于对照组,用药48h后再出血的发生率明显低于对照组,经统计学处理显示,差异有统计学意义(P<0.05)。结论奥曲肽治疗上消化道出血疗效优于奥美拉唑,值得临床推广和应用。

Objective To investigate the effect of octreotide in the treatment of upper gastrointestinal bleeding and compared with the effect of omeprazole. Methods Eighty cases of upper gastrointestinal bleeding treated from Jan.2007- Jan.2,010 were randomly divided into treatment group and control group,with 40 cases in each. The treatment group was treated with octreotide （Chengdu Tiantai Pharmaceutical Co.,Ltd.）,first 30mL dose 0.1mg＋saline intravenous injection,followed by 0.2mg ＋5% 500ml of glucose injection,intravenous infusion to maintain 12h for 2-3 days. The control group was treated with omeprazole（Jiangsu Kangjin edge Pharmaceutical Co.,Ltd.）, 40mg＋150mL normal saline infusion,two times per day. The clinical efficacy and hemostatic effect between the two groups were observed and compared. Results The total effective rate of the treatment group was 90.00%（31/40）,and that of the control group was 72.50%（29/40）. Chi-quare test showed a significant difference in the total effective rate between the two groups （P〈0.05）.Hemostatic time of the treatment group was significantly shorter than that of the control group, and the blood transfusion was significantly less than that of the control group,the re-bleeding rate was significantly lower in the treatment group 48h after medication than that of the control group,with a significant difference（P〈0.05）. Conclusion Octreotide is superior to omeprazole in the treatment of upper gastrointestinal bleeding,worthy of clinical promotion and application.
