大鼠重症急性胰腺炎肝损伤经胸导管引流干预时NF-κB、IL-6、TNF-α的表达及意义 被引量:12

摘要 目的探讨胸导管引流减轻大鼠重症急性胰腺炎肝损伤的作用及机制。方法采用经胰胆管逆行注射5%牛磺胆酸钠法制备大鼠急性胰腺炎模型。大鼠随机分为假手术(sham operation,SO)组、重症胰腺炎模型(severe acute pancreatitis,SAP)组、重症胰腺炎+胸导管引流(cervical thoracic duct drainage,DC)组,各组均在2、6、12h3个时间点进行观察。酶联免疫法检测大鼠血浆谷氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)的含量;以Western blot法检测各组动物在各时间点肝脏组织中核因子-κB(nuclearfactor-κB,NF-κB)p65蛋白表达情况。结果①3个时间点SAP组和DC组血浆ALT、AST、TNF-α和IL-6的含量均明显高于SO组,SAP组各指标均高于DC组(P<0.05);②2h时3组动物NF-κB p65表达SAP组和DC组已明显高于SO组(P<0.05);6h时SAP组、DC组NF-κB p65表达进一步升高,SAP组升高更为明显(P<0.01);12h时SAP组NF-κB p65表达又有增强,与DC组相比明显升高(P<0.05),DC组高于SO组(P<0.05)。结论胸导管引流对减轻重症急性胰腺炎合并的肝脏损伤有积极作用,其机制可能是胸导管引流可减少NF-κB的激活,使炎症因子TNF-α、IL-6分泌减少。 Objective To investigate the effects and molecular mechanism of the thoracic duct drainage on liver injury of severe acute pancreatitis (SAP). Methods Rats were randomly divided into sham operation group (SO), severe acute pancreatitis model group (SAP), thoracic duct drainage group (DC). Every group was divided into 3 subgroups according to time points of 2h, 6h and 12h. SAP rat models is prepared via retrograde injection of 5% sodium taurocholate to the pancreatic duct. The contents of alanine aminotransferase ( ALT), aspartate aminotransferase ( AST), tumor necrosis factor - α ( TNF - ct), interleukin - 6 ( IL - 6 ) in plasma were determined by enzyme - immunoassay method, and the expression of NF - κB p65 were observed by Western blot. Results ①Compared with the SO group, the contents of ALT, AST, TNF - α, IL - 6 were higher in DC group and SAP group ( P 〈 0.05 ), and the levels of these indexes in SAP group were higher than DC group ( P 〈 0.05 ). ② Stronger expressions of NF - κB p65 were found in SAP group and DC group than those in SO group at 2h( P 〈 0, 05) ; at 6h, NF - κB p65 of DC and SAP group further increased, and the level of SAP group was higher than DC group( P 〈 0.05) ; expression of NF - κB p65 was higher in DC and SAP group, and the two groups were both higher than SO group ( P 〈 0. 05 ). Conclusion The thoracic duct could decrease hepatic injury in SAP rats, and NF - κB plays an important role in this process. drainage
