FEMTO LDV飞秒激光制瓣LASIK术后像差的改变 被引量:9

Changes of ocular aberration after LASIK with FEMTO LDV femtosecond laser

摘要 目的对比飞秒激光及机械微型角膜刀制瓣行准分子激光原位角膜磨镶术(LASIK)后眼球像差的变化。方法选取接受FEMTO LDV飞秒激光制瓣患者38例(76眼)为试验组,同期接受Hansatome微型角膜刀制瓣患者35例(70眼)为对照组,制瓣后均行波前像差引导的准分子激光切削术。对2组波前像差进行比较。随访时间为3个月。结果试验组术后3个月时75眼(99%)达到或超过最佳矫正视力,与对照组66眼(94%)比较差异无统计学意义(P>0.05)。整体高阶像差试验组术后较术前无差异,对照组术后1周(0.52±0.30)μm、1个月(0.81±0.15)μm时较术前增加。同时术后各期2组间的差异均有统计学意义(均为P<0.05)。2组的球差均较术前增加,对照组球差的增加量高于试验组。垂直彗差试验组术后无明显变化,对照组较术前增加(P<0.05)。水平彗差2组均较术前增加(均为P<0.05)。结论飞秒激光制瓣LASIK可减少术后高阶像差的增加。 Objective To compare the aberration changes after LASIK with a mechanical microkeratome and femtosecond laser.Methods A total of 38 patients(76 eyes) underwent LASIK with the FEMTO LDV femtosecond laser(experimental group),and 70 eyes of 35 patients underwent wavefront-guided LASIK with the Hansatome microkeratome(control group).All cases were performed wavefront-guided LASIK.The high order aberration was compared after surgery.The follow-up time was 3 months.Results At 3 months after operation,UCVA were better than or equal to preoperative BCVA in 75 eyes(99%) of the experimental group and 66 eyes(94%) of control group,there was no difference(P〈0.05).The total high order aberrations were significantly increased at postoperative 1 week and 1 month in the control group,but not in the experimental group,there were statistical differences in each time point between two groups(All P〈0.05).The postoperative sphericallike aberrations in two groups were higher than that before operation,and the increases of the control group were higher than that of the experimental group.The postoperative vertical comatic aberrations increased in the control group(P〈0.05) and did not change in the experimental group.The postoperative horizontal comatic aberrations in two groups were higher that before operation,there were statistical differences(All P〈0.05).Conclusion LASIK with the FEMTO LDV femtosecond laser can reduce the increase of higher-order aberrations after operation.
