iNOS/NO体系在慢性肝炎患者及肝硬化患者中的表达比较

摘要 目的:探讨内源性NO在肝硬化造成门脉高压中的作用。方法:慢性肝炎组(20例)、肝硬化组(20例)及正常对照组(10例),采用硝酸还原酶法测定门静脉血、外周静脉血血浆中iNOS活力,所有受试者于B超下行肝穿刺术,取1.8cm×0.2cm肝组织2块,并用免疫组织化学法检测iNOS蛋白表达。RT-PCR方法观察肝组织iNOS蛋白及mRNA的表达。结果:正常对照组、慢性肝炎组与肝硬化组之间两两比较门静脉血及外周静脉血iNOS活力差别均有统计学意义(P<0.05或P<0.01)。正常对照组和慢性肝炎组的门静脉血与外周静脉血之间iNOS活力相比差异无统计学意义(P>0.05),肝硬化组门静脉血中iNOS活力高于外周静脉血(P<0.01)。慢性肝炎组及肝硬化组iNOSmRNA的相对表达高于正常对照组,肝硬化组高于慢性肝炎组(P<0.01)。慢性肝炎组及肝硬化组iNOS的灰阶值低于正常对照组,肝硬化组低于慢性肝炎组(P<0.01)。结论:iNOS/NO体系在肝硬化发生发展中起重要作用。 Objective: To investigate the levels of inducible nitric oxide synthase/nitric oxide (iNOS/NO)system in patients with chronic hepatitis and liver cirrhosis,and the effect of NO on cirrhotic portal hypertension thereof. Methods: The level of plasma NO was measured with colorimetric analysis. The expressions of iNOS and iNOS mRNA of liver tissues were measured by immunohistochemistry and RT-PCR. Results: The level of plasma iNOS was increased in portal and peripheral venous blood in patients with chronic hepatitis and cirrhosis than that in patients of control group (P 0.05 or P 0.01). Furthermore, the level of iNOS was increased in portal venous than that of peripheral venous in patients with liver cirrhosis(P 0.01). There were no significant differences in the levels of iNOS between portal venous and peripheral venous in both chronic hepatitis and control groups (P 0.05). The level of iNOS was significantly higher in portal venous than that of peripheral venous in patients with liver cirrhosis (P 0.01). The expression of iNOS mRNA was higher in patients with chronic hepatitis and liver cirrhosis than that of control (P 0.01), and was higher in patients with liver cirrhosis than that of patients with chronic hepatitis (P 0.01). The Grey values of iNOS were decreased in patients with chronic hepatitis and liver cirrhosis than that of control (P 0.01). The value was lower in patients with liver cirrhosis than that of patients with chronic hepatitis (P 0.01). Conclusion: iNOS/NO system has an important effect on the occurrence and development of hepatic cirrhosis.
