影响卵巢恶性生殖细胞肿瘤预后的多因素分析

Multivariate Analysis on Independent Factors Affecting the Prognosis of Malignant Ovarian Germ Cell Tumor

目的分析影响卵巢恶性生殖细胞肿瘤（MOGCT）预后的独立因素。方法用多因素分析方法分析我院1985年1月1日～2007年12月30日期间111例MOGCT患者的临床和随访资料。结果MOGCT临床病例的总1年，3年，5年，10年的生存率分别为86％，67％，56％，27％．手术-病理分期每增加1个期别，死亡相对危险度增加3．702倍。术后化疗疗程〉4个疗程的死亡相对危险度是≤4个疗程的68．1％．结论手术一病理分期、化疗疗程是决定MOGCT预后的独立因素。对MO—GCT做到早期诊断、早期手术治疗，以及术后给予正规、足量、及时的有效化疗是提高生存率和生存质量的首要方法。

Objective To analyze independent factors affecting the prognosis of malignant ovarian germ cell tumor（ MOGCT）. Methods Multivariate analysis was used to analyze the clinical characteristics and follow-up information of 111 cases of MOGCT admitted into our department during January 1985 and Dcember 2007. Results 1-,3-,5- and 10-years survival rate in ll 1 cases of MOGCT were respectively 86% ,67% ,56% and 27%. As operation-pathological stages increasing, the relative risk for death would increase 3. 702 times. The relative risk for patients with 4 courses of chemotherapy ＇after operation was 68.1% of patients with less than four courses of chemotherapy. Conclusion Independent factors affecting prognosis of MOGCT were operation-pathological stages and courses of post-operative chemothera- py. Early diagnosis and surgery,as well as regular, effective and timely chemotherapy after operation were the most methods for improving the prognosis of MOGCT.
