摘要
目的盆腔包虫病因缺乏特征性而易被误诊,特别是与良性卵巢囊腺瘤鉴别较困难,笔者通过多层螺旋CT(MSCT)平扫及增强扫描提高对盆腔包虫病和不典型良性卵巢囊腺瘤诊断的正确性。方法收集1999年至2009年经手术证实的30例盆腔囊性包块,年龄27~62岁,平均年龄39.5岁。所有病例均行MSCT平扫、增强扫描及多平面重建,并对其CT表现作回顾性分析。结果 5例女性盆腔包虫病,诊断正确率80%,其中2例为单囊型,3例为多囊型(其中1例可见一包虫囊肿破裂)。25例良性卵巢囊腺瘤诊断正确率88%,其中7例为单囊型,18例为多囊型。CT诊断定位正确27例(90%)。结论 MSCT对盆腔包虫病及良性卵巢囊腺瘤的定位、定性诊断及鉴别诊断具有较高的价值。
Objective To improve diagnosis accuracy of pelvic echinococcosis and non-typical benign ovarian cystadenoma by multi-slice spiral CT(MSCT) scanning.Methods From 1999 to 2009,30 cases of surgically confirmed cystic pelvic mass were enrolled,females,aged 27-62 years,mean age 39.5 years.All of them performed MSCT scanning,scan and multi-planar reconstruction,and the characteristics of CT were retrospectively analyzed.Results In five patients with pelvic echinococcosis,the diagnostic accuracy was 80 %,of those 2 cases were single capsule and 3 multi-cystic(1 case showed cyst rupture).In 25 patients with of benign ovarian cystadenoma,diagnostic accuracy was 88 %,7 cases of those were single capsule and 18 multi-cystic,the CT positioned diagnostic accuracy were 90 %(27 cases).Conclusion It is demonstrated that the MSCT has the excallent value in positioning and differential diagnosis of pelvic benign ovarian cystadenoma and echinococcosis.
