严重腹部闭合伤的临床诊治探讨

Clinical Diagnosis and Treatment of Severe Closed Abdominal Injury

目的:探讨严重腹部闭合伤的临床诊断和治疗方法。方法:回顾性分析我院自2006年1月～2009年12月收治的68例严重腹部闭合伤患者的临床资料。结果:本组68例患者手术治疗59例,保守治疗9例;治愈64例,死亡4例,死亡率为5.9%;死亡原因为失血性休克3例(肝破裂1例,肾破裂1例,脾破裂1例),多器官功能衰竭1例;并发症中发生肝性脑病者3例,切口感染者5例,败血症者2例,并发症发生率为14.7%。结论:腹部闭合伤是临床常见的危急重症之一,死亡率高、并发症多,在明确诊断后需尽快进行手术治疗。

Objective：To investigate the clinical diagnosis of closed abdominal injury and treatment.Methods：A retrospective analysis of our hospital from January 2006-December 2009 admitted 68 cases of severe closed abdominal injury in patients with clinical data.Results：68 patients,59 cases of surgical and conservative treatment in 9 patients;cured 64 cases,4 died,mortality was 5.9%;cause of death was hemorrhagic shock in 3 cases （1 case of rupture of liver,kidney rupture in 1 case,1 case of splenic rupture）,one case of multiple organ failure;complications occurred in 3 cases of hepatic encephalopathy,5 cases of wound infection, sepsis in 2 cases.complication rate was 14.7%.Conclusion：Closed abdominal injury is a common clinical emergency and severe one.the death rate.more complications,in the clear as soon as possible after diagnosis required surgical treatment.
