牙垫辅助屏气法经口插胃管的效果观察

Effects of Dental Pad Assisting Breath Holding in Oral-gastric Intubation

目的探讨对需要经口腔插管洗胃的患者,采用牙垫辅助屏气法留置胃管的效果。方法将180例患者按随机数字表分为对照1组、对照2组和观察组各60例。对照1组不用牙垫,在指导患者张口发"a"音的同时屏气,随即将胃管插入;对照2组应用口置牙垫的方法插管,不做屏气动作;观察组应用口置牙垫后指导患者张口发"a"音的同时做屏气动作,顺势将胃管插入。结果观察组插胃管1次成功率高,插管时间短、患者痛苦小、患者愿意接受,3组经比较差异有统计学意义(P<0.01)。结论对需要经口插管洗胃的患者,采用牙垫辅助屏气方法插管成功率高,插管时间短、患者痛苦小,有力配合了抢救,减轻了护理工作量。

Objective To explore the effects of dental pad assisting breath holding in patients undergoing oral-gastric intubation.Methods A total of 180 patients were randomly and equally divided into control group 1,control group 2 and observation group.The control group 1 did not employ dental pad and was guided to say ＂a＂ at the same time of breath holding,and thereupon received gastric tube intubation;the control group 2 received dental pad but did not hold their breath;the observation group was guided to say ＂a＂ and held their breath with the assistance of dental pad,then underwent oral-gastric intubation.Results Single intubation in observation group had a higher success rate,shorter intubation duration,less pain and higher patient willingness to receive.The results of the three groups were statistically significant（P0.01）.Conclusion For patients in need of gastric lavage,oral-gastric intubation with dental assisting breath holding is high in success rate,short in intubation duration and less in pain,which will strongly cooperate with the salvage and lower the nursing burden.
