甲型H1N1流感危重症机械通气患者不同卧位氧合效果比较 被引量:2

摘要 目的探讨不同卧位机械通气对甲型H1N1流感危重症患者氧合的影响。方法对23例甲型H1N1流感危重症急性呼吸窘迫综合征(ARDS)患者,入院即经气管插管术行呼气末正压通气(PEEP)呼吸机辅助呼吸,采取自身对照法行平卧位、半卧位、侧卧位、侧俯卧位、俯卧位各1h,循环6次;观察HR、MAP、潮气量(VT)、平均气道压(Pmean)、PaO2、静态顺应性(Cst)、氧合指数(OI)的变化。结果不同卧位患者各项指标比较,差异有统计学意义(均P<0.01),其中VT、PaO2、Cst、OI值俯卧位显著高于其他卧位,Pmean值显著低于其他卧位(均P<0.05)。结论俯卧位通气能有效改善甲型H1N1流感危重症患者的氧合状况,对血流动力学和呼吸力学无明显影响,侧俯卧位次之。
