^99Tc—MIBI亲肿瘤双时相显像诊断肺肿瘤的临床价值

摘要 目的探讨^99Tc-MIBI显像诊断肺癌的临床价值。方法用^99Tc-MIBI对30例临床诊断为肺肿瘤病人进行SPECT显像。结果肺癌与肺良性病变对^99Tc-MIBI的摄取和滞留差异均有显著性。对肺癌,以晚期摄取比值-S为判断标准,其诊断灵敏度、特异性及准确度分别为88%、80%和87%,以滞留指数-s为闽值,则分别为92%、100%和93%;^99Tc-MIBI诊断肺癌与病理学检查结果的符合率大于cT和MRI。结论^99Tc-MIBI显像对肺良恶性肿瘤的鉴别有较高的诊断价值,可以作为其他影像资料的有益补充。 Objective To evaluate the clinical value of ^99Tc-MIBI imaging for the diagnosis of lung neoplasms. Methods The SPECT imaging with ^99Tc-MIBI was used on 30 patients with pulmonary lesions. SPECT data were acquired 20 minutes and 2 hours after the injection of 740 - 1110 MBq ^99Tc-MIBI. The early ratio, delayed ratio and retention index(RI) were calculated. Results The uptake ratio and RI of ^99Tc-MIBI were different be- tween malignant and benign lesions. When the delayed ratio (-s) was 1.18 as threshold, the sensitivity, specificity and overall accuracy of ^99Tc-MIBI imaging for the malignant lesion was 88% ,80% and 87% , respectively, and there was no significant difference in different histological entities of the lung carcinoma( P 〉 0.05 ). The diagnosis concordance of ^99Tc-MIBI with the histologically diagnosed lung cancer was higher than that of CT and MRI. Conclusions ^99Tc-MIBI is a good imaging agent and is useful in differentiating malignant lung neoplasms from the bengin ones.
