甲状腺良、恶性结节超声弹性成像特征分析 被引量:70

摘要 目的评价超声弹性成像在甲状腺良、恶性结节鉴别诊断中的应用价值。方法对142例甲状腺结节患者的184个甲状腺结节进行常规超声及弹性成像检查,观察弹性成像特征,进行评分,并与病理结果对比。结果甲状腺良性结节弹性评分以0~2分多见,甲状腺恶性结节弹性评分以3~4分多见,良、恶性结节弹性评分差异具有统计学意义。以3分为鉴别甲状腺良、恶性结节的诊断界点,敏感度、特异度及准确率分别为85.71%、84.50%和84.78%。结论超声弹性成像在甲状腺良、恶性结节的鉴别诊断中具有很好的应用价值。 Objective To observe the value of ultrasound elastography (UE) in differentiating benign and malignant nodules of the thyroid. Methods A total of 142 patients with 184 thyroid nodules underwent conventional ultrasonography and elastography. The elastograms of 184 thyroid nodules were retrospectively scored and analyzed. The results of nodule scores were contrasted with pathology. Results The majority of the elasticity scores in benign thyroid nodules covered score 0 to 2, whereas most of the malignant thyroid nodules's scores covered 3 to 4. There was statistical significant difference between the two kinds of nodules. If the cut-off value of the elastographic scores was equal or more than 3, the sensitivity, specificity and accuracy was 85.71%, 84.50% and 84.78%, respectively. Conclusion UE is helpful in differentiating benign and malignant lesions of thyroid, and it is valuable in clinical practice.
