摘要 目的:探讨CT灌注成像在评价周围动脉闭塞性疾病方面的应用价值。材料和方法:采用64排螺旋CT对24例周围动脉闭塞性疾病患者和14例正常志愿者进行下肢CT灌注扫描,计算血容量(blood volume,BV)、血流量(blood flow,BF)、平均通过时间(mean transit ti me,MTT)和灌注达峰时间(ti me to peak,TTP)灌注参数,分析缺血组与对照组各灌注参数之间的差异。将周围动脉闭塞性疾病患者根据病情严重程度的Fontaine分级分成4组,分别对比4个组之间各灌注参数的差异。结果:缺血组BV、BF参数患病下肢均低于对照组(P<0.05)。缺血组临床分级各组之间BV值均存在显著差异(P<0.05),BF在Ⅰ级和Ⅲ级间差异有统计学意义(P<0.05)。MTT和TTP在各级别组间差异均无统计学意义(P>0.05)。结论:BV、BF值可作为CT灌注成像评价周围动脉闭塞性疾病病情轻重的参考指标。 Purpose To evaluate the clinical application of CT perfusion in peripheral arterial occlusive disease(PAOD).Materials and Methods Twenty-four patients with PAOD and 14 healthy volunteers underwent CT perfusion of lower extremities.The perfusion indexes of blood volume(BV),blood flow(BF),mean transit time(MTT)and time to peak(TTP)were obtained and compared between the patients and the controls.The patients were graded by Fontaine grade and compared between each two groups.Results The index of BV and BF were significantly higher in patients than in PAOD(P〈0.05).BV showed statistical difference between each two of the Fontaine grades(P〈0.05).BF had statistical difference between grade I and grade Ⅲ(P〈0.05).MTT and TTP had no difference between each two groups(P〈0.05).Conclusion The BV and BF of CT perfusion is useful in evaluating the severity of PAOD.
