直肠癌局部浸润3.0T磁共振征象与病理学T分期的对照研究

3.0T MR Image Signs of Local Infiltration in Rectal Cancer with Its Pathological T Staging:A Comparison Study

目的:探讨直肠癌局部浸润的MRI征象与病理T分期的关系。材料和方法:经病理证实的62例直肠癌患者,术前接受MR检查。使用3.0TMRI对直肠癌局部浸润征象进行分析、归类、做出分期诊断,并与术后病理组织学肿瘤T分期进行对照。结果:62例直肠癌MRIT分期总的诊断准确性为77.4%,≤T2～T4期MRI诊断准确性分别为88.71%、77.42%和88.71%。直肠癌局部浸润的MRI征象与病理T分期间相关性较强(r=0.725,P<0.001)。直肠癌侵犯肠周径程度与病理T分期呈中等正相关(r=0.699,P<0.001)。结论:MRI征象的正确识别和肿瘤对肠管周径侵犯程度的评价对直肠癌T分期诊断有一定帮助。体外相控阵线圈3.0TMRI技术可较准确地显示直肠癌的T分期,有助于计划手术方案以确保对病灶的完整切除,降低复发率。

Purpose To evaluate the relationship between the MRI signs of local infiltration and pathological staging of rectal cancer.Materials and Methods Sixty-two cases of pathologically confirmed rectal cancer underwent MR scan before operation.3.0T MR signs were analyzed and classified.Then the staging was made accordingly and compared with histopathological T staging.Results In 62 cases,the overall diagnostic accuracy of T staging was 77.4%,MRI diagnostic accuracy of ≤T2T4 staging was 88.71%,77.42% and 88.71%,respectively.The relationship between the MR signs of local invasion and the pathological T staging was a strong correlation positive（r=0.725,P〈0.001）.The relationship between circumference invasion of rectal cancer and the pathological T staging was A mild positive correlation positive（r=0.699,P〈0.001）.Conclusion Correct identification of MR signs and the evaluation of the extent of the circumference invasions of rectal cancer are conductive to the T staging of rectal cancer.3.0T MRI with a phased-array coil is an accurate imaging tool to evaluate T staging of rectal carcinoma.This technique is helpful in planning the surgery programme to ensure a complete removal of the lateral spread of the tumor and reduce local recurrence rates.
