摘要 目的探讨机械通气救治急性肺水肿的临床疗效,评价机械通气治疗急性肺水肿的价值。方法对24例急性肺水肿患者在常规药物同时应用机械通气治疗(采用面罩无创正压通气和(或)有创机械通气),记录机械通气治疗前、治疗后患者呼吸(R)、心率(HR)、血气分析。结果 24例患者临床症状明显改善,在机械通气2 h小时后呼吸(R)、心率(HR)下降,氧分压(PaO2)升高,二氧化碳分压(PaCO2)下降,机械通气前后,这些数值显著变化(P〈0.01)。20例救治成功,仅1例死于急性肺水肿。结论机械通气是治疗急性肺水肿的重要手段,把握时机及时应用,可提高救治成功率。 Aim To explore the curative effect and evaluate the ways of mechanical ventilation in treatment of patients with acute pulmo-nary edema.Methods 24 cases ses of patients with acute pulmonary edema received face mask positive-pressure ventilation and(or) in-vasive mechanical ventilation when being given conventional therapy.The variations of RR(respiration rate),HR(heart rate),ABG(ar-teaial blood gas) of pre-ventilation and post-ventilation were measuried after two hours.Results 24 patients’clinical symptoms im-proved obviously after mechanical ventilation.Respiration rate and heart rate were decreased.Partial pressure of oxygen in artery(PaO2) were increased.Partial pressure of carbon dioxide in artery were decreased after two hours.The parameters of ph,PaO2,PaCO2,HR and RR had significant difference(P〈 0.01) compared with those before ventilation.20 patients survived successfully,and only one patient died from acute pulmonary edema.Conclusion Mechanical ventilation is an effective method to treat acute pulmonary edema.If timely adopted it is possible for it to improve the survival rate.
