中药治疗的烧伤创面细菌培养前期质量控制研究

Prophase quality control of bacteriology inspection about the burn wounds treated by Chinese crude drug

目的探讨创面两种不同的标本采集方法和接种时间对接受中药创面治疗的同一烧伤创面细菌培养的影响。方法采用从创面表面分泌物和创面坏死组织两种方法从本组46例接受中药治疗的深度烧伤患者的同一创面同一时间采集标本,每组标本按采集后及时和后1 h进行接种,进行相同条件的细菌培养实验,比较各组革兰阳性球菌和革兰阴性杆菌检出率。获取的数据用SPSS-12软件进行分析处理。结果 （1）从坏死组织中采集标本组革兰氏阳性球菌检出率高于从创面分泌物采集标本组,其差异具有统计学意义,P〈0.05;从坏死组织中采集标本组革兰阴性杆菌检出率略高于从创面分泌物采集标本组,其差异无统计学意义P〉0.05）。（2）从创面表面分泌物采集标本及时接种组革兰阳性球菌检出率高于后1 h接种组,其差异有统计学意义（P〈0.05）,其余各组差异无统计学意义。结论从创面坏死组织采集标本且采集后及时接种较从创面分泌物采集标本进行细菌培养具有更高的敏感性。

Aim To investigate the effect of different approach of specimen collection and times of transfer of culture on the bacterial cul-ture about the burn wounds treated by Chinese crude drug.Methods 46 patients with deep burn wounds treated by Chinese crude drug were selected to be the objects for this research.Specimens were collected from secertion on the surface of the wounds,and from the nec-rotic tissues deeply in the wounds.They were applied for bacterial culture at the same time.All the specimens were inoculated immediate-ly and 1 hour later,and bacterial culture was conducted at the same conditions,then the difference between the two approaches were compared by investigate the times of transfer of culture about the detection rates of gcoccobacteria and gbacillus.All the data were ana-lyzed with SPSS-12.Results Either inoculated immediately or inoculated 1 hour later,there was a higher detection rate of gcoccobacteria from the specimens collected from the necrotic tissues deeply in the wounds than from secretion on the surface of the wounds,and the difference showed statistical significance（P〈 0.05）.The difference of the detection rates of gbacillus from two kinds of specimens showed no statistical significance（P 〉0.05）.For the specimens collected from secretion on the surface of the wounds,there was a higher detection rate of gcoccobacteria inoculated immediately than that inoculated 1 hour later,and the difference showed statistical significance（P〈 0.05）.The other differences showed no statistical significance（P 〉0.05）.Conclusion In bacteriology inspection about the deep burn wounds treated by Chinese crude drug,the approach of collecting specimen from the necrotic tissues deeply in the wounds and inoc-ulating immediately has a higher detection rate than the others.
