摘要
目的分析新疆塔城及博乐地区残联组织的大批量集中的白内障手术中特殊病例15例(15眼)的处理方法和结果。方法应用手法小切口白内障摘出术。(1)晶状体半脱位6例,应用U形前囊撕开,晶状体核摘出注吸皮质完成后人工晶状体植入。(2)重度驼背7例,手术台V形摺叠,患者头后仰施术。(3)冠状动脉支架术后1例,术中球后出血并发急性青光眼,于术后第1天全麻白内障摘出人工晶状体植入。(4)精神高度紧张1例,术中憋气眶压过高,黏弹剂不能注入前房,术中给基础麻醉后完成手术。结果晶状体半脱位6例中4例植入人工晶状体,2例囊内摘出;并发青光眼1例并继发心肌梗塞,抢救成功;精神高度紧张1例应用基础麻醉后手术完成。本组15例术后视力0.3～0.7(包括矫正视力)以上。结论残联组织的以医疗小组形式依托当地医院手术。因大批量集中手术,须详细筛查,对特殊的和高难度的病例,术前进行细致检查,术中精心操作,也能获得较好效果。但残联筛选病例要严格,以免手术后患者视力不改善或其它意外,给慈善行动带来医疗纠纷。
Objective The treatment and analysis of 15 cases complex cataract cases that cooperate with Tacheng and Bole disabled league in Xinjiang. Methods All cases complete with small incision ECCE. That include partial lens dislocation 6 cases,severe kyphosis 7 cases,post operation with coronary artery bracket 1 case and excessive tension 1 case. Results Visual acuity of 15 cases was better than 0. 2. Conclusions The large numbers of cataract surgery that cooperate with disabled league should be complete in a little time. So before-operative filtration must be strict and particular. Complex cataract should be excluded. That helped to reduce dissension in medicine treatment.
