经尿道前列腺电切术致膀胱及前列腺包膜穿孔18例诊治分析

Diagnosis and Treatment of Capsular Perforation of Transurethral Resection of Prostate

目的探讨经尿道前列腺电切术(TURP)致膀胱及前列腺包膜穿孔的诊治与预防措施。方法回顾性分析我院2001—2009年530例行TURP的良性前列腺增生症患者的临床资料,分析TURP致膀胱及前列腺包膜穿孔的诊断和治疗措施。结果 18例患者发生膀胱及前列腺包膜穿孔,通过正确判断包膜穿孔的类型和程度,给予相应治疗,均痊愈出院。结论 TURP过程中如出现膀胱及前列腺包膜穿孔,需正确诊断和及时处理。熟练操作、止血彻底、视野清晰及解剖结构清楚能明显降低穿孔的发生率。

Objective To explore the experience of diagnosis,therapy and prevention of perforation of prostate and bladder capsule caused by transurethral resection of prostate（TURP）.Methods We retrospectively analyzed 530 routine clinical data of TURP patients our hospital from 2001 to 2009 and analyzed the experience of diagnosis,therapy and prevention of perforation of prostate and bladder capsule caused by TURP.Results 18 cases of patients with prostate and bladder capsule perforation which were judged correcyly the type and extent of capsule perforation and were given corresponding treatment were all cured.Conclusion Correct diagnosis and timely treatment must be taken,if prostate and bladder capsule perforation occurs when TURP operation in progress.Skillful operation,less haematoma,clearview and normal anatomic structures can decrease the incidence of perforation.
