摘要
目的观察ω-3鱼油脂肪乳对呼吸机相关性肺炎(VAP)的临床疗效。方法 56例VAP患者随机分为观察组30例和对照组26例,均行气管切开插管,应用有创呼吸机辅助通气,对照组给予常规治疗;观察组在常规治疗的基础上加用ω-3鱼油脂肪乳静脉滴注。观察2组疗效。结果观察组有效率为90.0%高于对照组的65.4%,观察组机械通气时间为(79.8±12.3)h短于对照组的(93.2±16.4)h,差异均有统计学意义(P<0.05)。观察组脱机成功率为93.3%(28/30)高于对照组的84.6%(22/26),差异无统计学意义(P>0.05)。观察组治疗后血浆D-乳酸、内毒素水平均显著低于治疗前(P<0.05),对照组治疗后血浆D-乳酸及内毒素水平与治疗前比较差异无统计学意义(P>0.05),观察组治疗后D-乳酸及内毒素水平均低于对照组(P<0.05)。结论ω-3鱼油脂肪乳对呼吸机相关性肺炎有良好辅助治疗作用。
Objective To observe the efficacy of ω-3 fish oil fat emulsion on ventilator associated pneumonia(VAP).Methods 56 cases were randomly divided into observation group of 30 and control group of 26.Both groups were given tracheotomy tube and mechanical ventilation by invasive respirator.The control group was given traditional treatment,while the observation group basing on the control group was added ω-3 fish oil fat emulsion iv.Observed the effect of 2 groups.Results The effective rate of the observation group was 90.0% higher than 65.4% of the control group,while the duration of mechanical ventilation was (79.8±12.3)h shorter than (93.2±16.4)h of the control group,and the differences were statistically significant(P0.05).The off-line successful rate of the observation group was 93.3% higher than 84.6% of the control group,but the difference was of no statistical significance(P0.05).In the observation group,the D-lactic and endotoxin level were significantly lower than those before treatment(P0.05),while there were no significant differences in the control group(P0.05).After treatment,the D-lactic and endotoxin level of the observation group were significantly lower than those of the control group(P0.05).Conclusion ω-3 fish oil fat emulsion is a good adjuvant therapy for VAP.
