真皮下血管网皮瓣在手指热压伤修复中的应用

Application of subdermal vascular network in repairing fingers trauma of heat pressing hurt

目的总结真皮下血管网皮瓣修复手指热压伤创面的效果。方法应用胸腹部真皮下血管网皮瓣修复手指热压伤后皮肤软组织缺损创面48例。结果术后43例皮瓣成活,5例皮瓣远端小部分坏死,经换药后愈合;术后随访1～3年,受区皮肤平整、外形美观、色泽好、质地软,耐磨性好,功能恢复满意。结论应用胸腹部真皮下血管网皮瓣修复手指热压伤后皮肤软组织缺损具有术后手外形好、功能恢复快、断蒂后无需二次修整等优点,是比较理想的一种修复方法。

Objective To summarize the the clinical experience in the treatment of fingers injuries by heat pressing with subcutaneous vascular net flaps. Methods Subcutaneous vascular net flaps from the abdomen or thorax were applied to reconstruct the defective derma and parenchyma of fingers injured by heat pressing hurt in 48 cases. Results In 43 cases of all, the skin flaps survived totally. A few distant parts of skin flaps in 5 cases necrosed, but were cured by some times of changing fresh dressing soon. Following up for 1 -3 years, the repair result was satisfactory with good appearance in color, flexibility and abrasion resistance. Conclusion The subcutaneous vascular net flaps might be effective method in treatments of soft tissue defects of fingers damaged by heat pressing hurt and many advantages could be obtained such as the satisfactory cosmetic and functional result, avoidance of secondary repairing after pedicel division, etc.
