老年2型糖尿病患者实施系统性健康教育的效果

Effects of Systemic Health Education on Decreasing Medical Expenses of Elderly Patients with Type 2 Diabetes Mellitus

目的探讨系统性健康教育在老年2型糖尿病患者中应用的效果。方法对2008年2月至2009年2月在内分泌科门诊治疗的108例老年2型糖尿病患者进行系统性健康教育,干预时间为1年,分别比较干预前后患者的代谢控制水平、自我管理能力及医疗费用支出情况。结果系统性健康教育后,患者的空腹血糖、血压、LDL-C及HbAlc均明显改善,与干预前差异有统计学意义(P<0.01);患者在主动监测血压、饮食控制、合理运动、自我足部护理和门诊复诊等方面均有显著改善,与干预前差异有统计学意义(P<0.01);患者年均医疗费用支出由(4188.08±2852.32)元下降为(3458.97±2074.20)元,差异有统计学意义(P<0.05)。结论系统性健康教育有助于提高老年2型糖尿病患者的自我管理能力,从而更好地控制血糖、血压、血脂及糖化血红蛋白水平,减少并发症的发生,降低医疗费用支出。

Objective To evaluate the effects of systemic health education on the medical expenses of elderly patients with type 2 diabetes mellitus（T2DM）.Methods The systemic health education was conducted in 108 elderly patients with T2DM in endocrine outpatient department from February 2008 through February 2009.After one year of intervention,comparisons on metabolic control level,self-management ability and disbursement situation of medical expense were performed before and after the intervention.Results After the systemic health education,the fasting blood-glucose（BG）,blood pressure（BP）,LDL-C and HbAlc were significantly improved than those before the intervention（P0.01）;Significant improvements were also observed on active BP monitoring,alimentary control,sensible exercise,self foot nursing and subsequent visit（P0.01）.The average annual expenses had decreased from RMB（4188.08±2852.32）to RMB（3458.97±2074.20）with significant difference（P0.05）.Conclusion Systemic health education can improve the self-management ability of T2DM patients and thus better control BG、BP、blood fat（BF） and glycosylated hemoglobin levels.Meanwhile,it can reduce the occurrence of complications and decrease the medical expenses.
