摘要
目的:探讨胃癌病人术后早期肠内营养的免疫调节作用。方法:40例胃癌术后随机分为全肠外营养(TPN)组和早期肠内营养(EEN)组。术前1天、术后第2天和第8天测定免疫指标和急性炎性指标;观察术后并发症、住院时间。结果:研究期间无死亡,无严重并发症,研究结束时,EEN组外周血CD3+、CD4+、CD4+/CD8+比值明显高于TPN组;IL-6明显低于TPN组;EEN组住院时间短于TPN组。结论:胃癌病人术后早期肠内营养能改善病人的免疫功能,减轻炎性反应,缩短住院时间。
Objective:To evaluate the immune regulation effect of early postoperative enteral nutrition in patients with gastric cancer.Methods:On the second postoperative day,40 patients were randomly divided into TPN group and EEN group.All variables were measured before operation and on postoperative day 2 and 8.Blood was drawn to assess immunoglobins,T-lymphocyte subsets, NK cells, C-reactive protein,IL-6 and TNF-α.Morbidity,mortality and length of hospital stay were also evaluated.Results:Neither mortality nor morbidity occurred during the period of study.After the study,the levels of total lymphocytes,CD3+,CD4+ and CD3+/ CD8+ ratio in the EEN group were significantly higher than the levels in the TPN group.The concentrations of IL-6 and the length of hospital stay in the EEN group were significantly lower than in the TPN group.Conclusion:Early postoperative enteral nutrition in patients with gastric cancer has the benefit effects of enhancing the immune function,reducing the inflammatory response and the length of hospital stay.
