真菌性角膜炎流行病学分析

Epidemiologic Analysis on Fungal Keratitis

目的分析真菌性角膜炎患者的流行病学特征。方法对2005年1月至2009年12月经培养确诊为真菌性角膜炎的84例患者进行回顾性分析,对其流行病学特征进行分析。结果真菌性角膜炎发病呈上升趋势,年发病高峰集中在9月和10月。男性患者约占63.10%,年龄在51～70岁患者占所有患者的61.90%,94.05%为农民,79.76%患者有明确外伤史,其中63.10%为植物性外伤,最终行手术治疗患者数为23例(27.38%),其中行溃疡清创联合结膜瓣遮盖术12例(14.29%),溃疡清创联合羊膜覆盖结膜瓣遮盖术6例(7.14%),穿透性角膜移植术5例(5.95%)。结论真菌性角膜炎是严重的致盲性眼病,其发病率呈上升趋势,应提高对该病的认识和诊断水平,对具有患病危险因素的患者应进行积极的抗真菌治疗。

Objective To analyze epidemiologic features of fungal keratitis.Methods The retrospective analysis was conducted for 84 cases of fungal keratitis diagnosed through culture from January 2005 to December 2009,focusing on the analysis of epidemiologic features of disease.Results It was discovered that the incidence of fungal keratitis increased in tendency and the peak incidence in a year was in September and October.The male patients accounted for 63.10%.The patients aged from 51 to 70 years accounted for 61.90%.94.05% of patients were peasants.97.76% of patients had definite traumatic injury history,of which,63.10% of the cases were vegetative traumatic injury.23 cases（27.38%）had received surgery eventually,of which,12 cases（14.29%）had ulcer debridement combined conjunctival flap,6 cases（7.14%）had ulcer debridement and amniotic membrane covering conjunctival flap and 5 cases（5.95%）had penetrating keratoplasty.Conclusion Fungal keratitis is the severe blind-inducing eye disease and its incidence increases in tendency.Hence,it is required to enhance the recognition of the disease and improve diagnostic level.The active anti-fungal treatment should be administered for the patient with the risk of this disease.
