摘要
为探讨影响中老年Ⅱ型糖尿病患者血清甲状腺激素变化的因素及其意义, 测定55 例中老年Ⅱ型糖尿病人血清促甲状腺激素( T S H) 、3 , 5′, 3′- 三碘甲状腺原氨酸( T3) 、甲状腺素( T4) 、游离 T3 ( F T3) 、游离 T4 ( F T4) 及3 , 3′, 5′- 三碘甲状腺原氨酸 (r T3) , 并进行上述指标与年龄、病程、糖化血红蛋白( G Hb) , 胰岛素及 C 肽的相关分析. 结果: 糖尿病组 T3 , F T3 , F T4 低于对照组 ( P 分别为< 0001 , 0001 ,005) , T3 的降低与年龄、病程、 G Hb 呈负相关 ( r 分别为- 038 , - 050 , - 036 ; P< 001 , 001 ,005) , F T3 , F T4 降低与 G Hb 呈负相关 (r 分别为- 037 , - 050 , P< 005 、001) . 结果表明: 中老年Ⅱ型糖尿病患者伴有血清甲状腺激素的变化, 而导致这种变化的机制可能是通过不同的环节进行的. 这种甲状腺激素的变化与高血糖及其持续时间密切相关, 血清甲状腺激素的测定可作为Ⅱ型糖尿病患者代谢紊乱控制与否的一个指标.
To probe into the significance of serum thyroid hormone changes and its influencing factors in middle and aged patients with type Ⅱ diabetes, serum thyroid stimulating hormone (TSH), triiodothyronine(T 3 ), L-thyroxine (T 4 ), free T 3, free T 4 (FT4) and reverse T 3 (rT 3 ) were measured with radioimmunnoassay in 55 middle and aged patients of type Ⅱ diabetes. Serum T 3, FT 3 and FT 4 levels were significantly lower in diabetic patients than those in controls ( P <0 001, 0 001, 0 05 respectively). The decreased serum T 3 was negatively correlated with the age, disease course and glycosylated hemoglobin(GHb) concentration of the patients ( r =-0 38, -0 50, -0 36, P <0 01, 0 01, 0 05 respectively). The decreased serum FT 3 , FT 4 were negatively correlated with GHb content ( r = -0 37 ,-0 50, P <0 05, 0 01 respectively). The results showed that type Ⅱ diabetics in the middle and elderly are usually associated with changes of serum thyroid hormone resulting from more than one mechanism and related closely to hyperglycemia and its duration. The measurement of serum thyroid hormone can be used as an indicator suggesting the degree of metabolic disorder in type Ⅱ diabetics.
