改良保留子宫动脉上行支子宫切除术的临床应用

Application of Improved Hysterectomy with Intact Ascending Uterine Artery

目的:研究保留子宫动脉上行支子宫体大部切除术临床应用价值,初步探讨对卵巢功能的影响。方法:选择同期行子宫切除术198例患者,随机分成两组,经腹改良保留子宫动脉上行支子宫体大部切除术98例为治疗组(改良组),经腹子宫次切术100例为对照组(对照组),比较两组手术时间、术中出血、术后病率、住院时间、术后复发、术后宫颈病变及术前、术后1、3、6月血FSH、LH、E2值等。结果:通过比较,两组手术时间、术中出血、术后病率、住院时间无明显统计学差异,但在术后1、3、6月血FSH、LH、E2值、子宫病变复发率及宫颈病变出现率上有明显统计学差异(P<0.01)。结论:保留子宫动脉上行支子宫体大部切除术对卵巢功能有一定保护,尤其是改良组宫颈病变出现率低,受到患者青睐。

Objective：To study clinical application value of subtotal hysterectomy with intact ascending uterine artery and discuss its influence to ovary function.Method：198 patients needing hysterectomy were randomly divided into treatment group and control group.98 patients in treatment group were treated with improved subtotal hysterectomy with intact ascending uterine artery,and 100 patients in control group were treated with abdominal subtotal hysterectomy.The operation duration,intra-operative bleeding,hospitalization duration,postoperative recurrence and cervix diseases,as well as FSH,LH,E2 blood levels at the first,third and sixth month after surgery were compared between two groups.Result：There were no significant differences between two groups in operation duration,intra-operative bleeding,and hospitalization duration,while there were significant differences in FSH,LH and E2 blood levels,recurrence rate of uterus and cervix diseases.Conclusion：Improved subtotal hysterectomy with intact ascending uterine artery can protect the ovarian function,especially can protect from cervix diseases.As a result,it is favored by the patients.
