血必净对急性胰腺炎患者血浆内皮素浓度的影响 被引量:2

摘要 目的:观察血必净对急性胰腺炎(AP)患者血浆内皮素(ET)浓度的影响,从而了解血必净对微循环的改善作用。方法:选择54例AP患者,其中,轻症急性胰腺炎(MAP)30例,重症急性胰腺炎(SAP)24例。将患者随机分为常规+血必净治疗组(A组)和常规治疗组(B组),各组均含SAP12例、MAP15例。同时,以健康志愿者20例作为对照组。以放射免疫法(RIA)测定血浆ET浓度。结果:重症急性胰腺炎患者血浆ET浓度为(153.47±9.52)ng/L,显著高于轻症急性胰腺炎患者的(90.71±10.34)ng/L(P<0.05)和对照组的(51.78±7.45)ng/L(P<0.05)。B组患者经常规治疗后ET水平与治疗前比较无显著性差异(P>0.05),A组治疗后血浆ET水平较治疗前明显下降[(63.37±8.54)ng/L,(108.56±9.21)ng/L](P<0.05)。A组患者平均住院天数为11.73±1.89d,明显短于B组的16.22±2.15d(P均<0.05)。结论:血必净注射液治疗AP可有效降低患者血浆中的ET含量,其对改善AP患者的微循环具有较好的效果。 Objective:To explore the changes of plasma endothelin(ET) as well as the effects of Xuebijing injection on themicrocirculation of patients with acute pancreatitis (AP).Methods:A total of 54 patients with AP was divided into group A (n=27)and B (n=27),treated with routine method(groupB) and Xuebijing injection added routine method (group A),respectively.Another20 normal individuals were used as normal controls.The levels of plasma ET were measured by radioimmunoassay.Results:The levels of ET in patients with severe acute pancreatitis[(153.47±9.52)ng/L] were significantly higher than those in the cases with mild acute pancreatitis[(90.71±10.34)ng/L,P〈0.05] and normal controls[(51.78±7.45)ng/L,P〈0.05].After treatment,the levels of ET in group B were slightly decreased compared with those before treatment(P〉0.05),while the levels of ET in group A were obviously decreased compared with those before treatment[(63.37±8.54)ng/L vs (108.56±9.21)ng/L,P〈0.05].The average in-hospital time(11.73±1.89d) of group A were significantly shorter than that of group B (16.22±2.15d,P〈0.05).Conclusions:Endothelin is involved in the occurence and progress of microcirculation disorder in pathogenesis of AP.Xuebijing injection is effective in the treatment of AP.
