摘要
目的:与手术诊断对比研究彩色多普勒血流显像诊断室间隔缺损的价值。方法:应用二维超声(2DE)及彩色多普勒血流显像(CDFI)联合检查148例室间隔缺损(VSD)患者,选择过隔血流束最清晰、最宽切面、取左室面于VSD起始部测量其直径,进行定性、定位及定量分析。结果:148例术前超声诊断147例,符合率99.3%;定位符合率93.2%;超声定量测值在嵴上型与手术测值接近,而在嵴下型及隔瓣下型均较手术所见小。超声对合并症的诊断正确率达83.0%。结论:2DE和CDFI联合检查可显示VSD的部位、大小、形态,并可直观清晰显示VSD分流方向,同时对合并症的诊断亦有价值,是一种可靠的非创伤性术前诊断方法。
To explore the value of color doppler flow imaging (CDFI) in diagnosis of ventricular sep-tal defects (VSD). Methods: 148 patients with VSD were diagnosed by combin ation of two dimensional echog-raphy (2DE) and CDFI, the across-septum flow with clearest and widest views were selected, and the diameters at the beginning of the left ventricular view were measured for analysis. Results: 147 cases out of 148 were diag-nosed pre-operatively by echography, and the agreement rate between echography and operation reached 99. 3 % ; the agreement rate for the location of VSD between echography and operation was 92. 6% ;the mea-surement values obtained by echography were close to those at supracristal ventricular septal defects during op-eration, but smaller than those at subscrital ventricular septal defects and submembranous ventricular septal de-fects; furthermore, the diagnostic rate by echography were up to 83.0%. Conclusions: combin ation of 2DE and CDFI is efficient in detection of VSD, and the direction of flow can also be viewed distinctly and directly- These findings imply that echography is a re1iable noninvasive technique to diagnosie VSD pre-operatively.
