异丙酚的麻醉强度测定

摘要 目的:观察异丙酚对安氟醚最低肺泡有效浓度(MAC)及手术切皮无应激反应的MAC(MAC-BAR)的影响。方法:ASAI-Ⅱ级择期手术病人30例,随机分为两组,以手术切皮为疼痛刺激方法。异丙酚用3mgkg-1的诱导剂量加53.8μgkg-1min-1连续输注。结果:测得安氟醚MAC为1.609Vol%,MAC-BAR为1.736Vol%,与经典值相对照,异丙酚对安氟醚MAC没有明显影响,但能降低MAC-BAR35.5%。结论:异丙酚镇痛作用很弱或没有镇痛作用,异丙酚应用后不必用高浓度吸入麻醉药即可阻断循环与内分泌反应,用MAC-BAR作麻醉强度指标,能显著减少或基本消除手术中的心血管及神经内分泌应激反应。 Objective: To investigate the effects of propofol on the minimum alveolar concentration (MAC) and MAC to blocking adrenergic responses (MACBAR) of enflurane to determine the anesthetic potency of propofol. Methods: Thirty patients were equally randomized into two groups. The initial surgical skin incision was used as the standard noxious stimulus for determination of MAC. Propofol with the dose of 3 mgkg-1 was injected and then continuously infused at a dose of 53.8 gkg-1min-1. Results: MAC and MACBAR of enflurane were 1.609Vol% and 1.736 Vol% respectively. Compared with those previously reported, propofol could not significantly influence MAC of enflurane but decrease MACBAR by 35.5%. FZConclusion: Propofol has no effects on enflurane's MAC but helpful for blocking the cardiovascular and neuroendocrine responses.
