大剂量皮质类固醇激素联合环磷酰胺治疗重症天疱疮30例

Large doses of corticosteroid in combination with cyclophosphamide in the treatment of 30 cases of severe pemphigus

采用大剂量皮质类固醇与环磷酰胺同时并用治疗重症天疱疮３０例。结果表明采用泼尼松相当量为６０ｍｇ／ｄ时控制病情者１２例，８０～１００ｍｇ／ｄ者８例，１２０～１５０ｍｇ／ｄ者６例，１８０ｍｇ／ｄ者１例，超过１８０ｍｇ／ｄ者３例。副作用包括血压升高、消化道出血、感染及精神症状，以上副作用经对症处理及皮质类固醇的迅速减量而减轻或消失。２６例患者近期痊愈，４例近期基本痊愈。结果证实联合用药具有疗效高，皮质类固醇减量迅速的优点，远期效果正在随防中。

Thirty severe patients with pemphigus were treated with large doses of corticosteroid in combination with cyclophosphamide. The results showed that twelve patients were controlled with prednisone equivalent 60mg/d, eight with 80mg/d, six with 120～150mg/d, one with 180mg/d and three with over 180mg/d. The side effects included high blood pressure, gastroinstestinal hemorrhage, diabetes mellitus, infections and psychic symptoms. All of the side effects were controlled and disappeared when treated accordingly and as the doses of corticosteroid were tapped. Twenty six patients were cured and four were basically cured in the hospital. The study confirms that the combined regimen is highly effective for severe pemphigus. The long term therapeutic effect of this regimen is under following up.
