腹腔镜技术在卵巢癌再分期手术中的应用(附22例报道)

Aplication of laparoscopico operation staging in early-stage ovarian cancer (a report of 22 cases)

目的探讨采用腹腔镜进行卵巢癌的再分期手术的可行性和安全性。方法对在外院初次手术后转诊至该科及在该科初次手术的早期卵巢癌和输卵管癌患者22例行腹腔镜卵巢癌补救性再分期手术,其中有17例患者在外院初次手术后转入该院行补救性再分期手术。结果手术顺利,手术时间120～180min,平均143.57min。术中出血量80～150mL,平均110mL。每例切除盆腔淋巴结(PLN)10～22个,平均16个,主动脉旁淋巴结(PALN)1～9个,平均5个。术中术后均未输血,无1例中转开腹及手术并发症发生。结论腹腔镜在早期卵巢癌再分期手术的应用,尤其是补救性再分期手术的应用有其实际的临床意义,但是有它的局限性,作为一种处理问题的手术方式选择使用还是安全可行的。

【Objective】To investigate the role of laparoscopic surgical staging in early stage ovarian cancer. To explore the safety and effectiveness of laparoscopic operation as treatment for surgical staging of ovarian carcinoma. 【Methods】Underwent laparoscopic surgical staging was performed in 22 patients of early-stage ovarian cancer,17 patients underwent laparoscopic salvable surgical staging.【Results】The operation was completed under laparoscope in 22 cases. All the operations were successfully completed and no complications were observed. The operation time was 120～180 min （mean,143.57 min）; the intraoperative hemorrhage volume was 80～150mL （mean,110 mL）; the number of excised pelvis lymph nodes was 10～22 （mean,16） and the number of excised paraaortic lymph node was 1-9 （mean,5）. 【Conclusion】Laparoscopic surgical staging in early stage ovarian cancer is safe,feasible.
