前牙铸瓷全冠、金铂合金烤瓷全冠与镍铬合金烤瓷全冠临床效果评价

Clinical Evaluation of Ceramic Crown,Gold-Platinum-Palladium Alloy Porcelain-Fused-to-Metal Crown and Nickel-Chromium Alloy Porcelain-Fused-to-Metal Crown on Anterior Teeth

目的观察E.MAX三代CAD-CAM铸瓷全冠、金铂合金烤瓷全冠与镍铬合金烤瓷全冠辅以纤维桩、树脂核,修复前牙残根、残冠的临床效果。方法对240例需桩核修复的前牙,经完善根管治疗后,以纤维桩、树脂核修复,并分别制作铸瓷全冠、金铂合金烤瓷全冠与镍铬合金烤瓷全冠各80件,进行多项指标2年临床的追踪观察。结果铸瓷冠在颜色方面优于金铂合金和镍铬合金烤瓷冠(P<0.05);金铂合金烤瓷冠在边缘密合度方面优于铸瓷冠和镍铬合金烤瓷冠(P<0.05);镍铬合金烤瓷冠在颜色、边缘着色、牙龈健康状况方面不及铸瓷冠和金铂合金烤瓷冠(P<0.05)。结论 E.MAX三代CAD-CAM铸瓷全冠在美学效果方面优于其它两种修复体;金铂合金烤瓷全冠在边缘密合度方面优于其它两种修复体,两者在牙龈健康和边缘着色方面也优于镍铬合金烤瓷冠。

Objective To evaluate the clinical effects of E.MAX third-generation CAD-CAM ceramic crowns,gold-platinum alloy porcelain-fused-to-metal crowns,nickel-chromium alloy porcelain-fused-to-metal crowns by fiber post and resin core on anterior prosthesis. Methods Totally 240 teeth were selected to receive root canal treatment.All teeth were restored with fiber post and resin core.80 anterior teeth were restored with E.MAX third generation of CAD-CAM all-ceramic crowns,80 anterior teeth were restored with gold-platinum alloy PFMC,and 80 anterior teeth were restored with nickel-chromium alloy PFMC.The indexes of the teeth were followed-up to 2 years. Results Ceramiccrown was significantly better than gold-platinum alloy PFMC and nickel-chromium alloy PFMC in color（P0.05）.Gold-platinum alloy PFMC was significantly better than ceramic crown and nickel-chromium alloy PFMC in marginal fitness（P0.05）.Ceramic crown and gold-platinum alloy PFMC were significantly better than nickel-chromium alloy PFMC in color,cervical margin staining,and gingival health（P0.05）. Conclusions E.MAX third generation of CAD-CAM all-ceramic crown is superior to gold-platinum alloy PFMC and nickel-chromium alloy PFMC in color.Gold-platinum alloy PFMC is superior to ceramic crown and nickel-chromium alloy PFMC in marginal fitness,while ceramic crown and gold-platinum alloy PFMC are significantly better than nickel-chromium alloy PFMC in gingival health and cervical margin staining.
