超声造影增强技术在探索穿支皮瓣血管中的应用 被引量:24

摘要 目的应用超声及超声造影增强技术(CEUS)检查并定位穿支皮瓣血管,为临床提供更为准确的参考依据。方法在彩超基础上运用CEUS对32例穿支皮瓣移植术患者行穿支血管检查,并行超声三维重建,除定位穿支血管出肌点外,力求清晰、直观地显示穿支与主干的关系以及穿支在皮瓣内的分支、分布情况。结果 31例(97%)穿支皮瓣血管经CEUS能更清晰地显示和定位,26例(81%)获得满意的三维重建图像。术中证实超声所见,术后皮瓣均存活。结论 CEUS能更加清晰、准确地显示和定位穿支血管,能在皮瓣血管检查中发挥重要作用。三维超声能直观地为临床手术提供穿支血管信息。 Objective To detect the vessel perforators of skin flap with ultrasonography and contrast-enhanced ultrasound(CEUS) to provide more accurate images for clinical applications.Methods Totally,32 subjects of perforator flaps were studied by CEUS and 3D ultrasound.In addition to the position where the blood vessel perforates from muscle into adipose layer,the research emphasized to display the relationship of perforator and its source vessel,and to show its branches and distribution in the flaps.Results Among them,31 subjects(97%) were observed more clearly with CEUS than conventional ultrasound and 26 subjects(81%) could be displayed wonderfully in 3D ultrasound.Perforators which were detected by ultrasound were confirmed in the surgery and the transferred flaps survived completely.Conclusion Perforators can be displayed more clearly and located more accurately by CEUS,which plays a very important role in perforator flap.Three dimentional ultrasound can display excellent images with the information of perforators for clinical surgery.
