CAD/CAM氧化锆烤瓷后牙固定桥与常规烤瓷固定桥的临床应用比较研究 被引量:22

摘要 目的比较观察CAD/CAM氧化锆与传统的镍铬合金、金合金烤瓷固定桥的临床效果,为评价CAD/CAM氧化锆烤瓷后牙固定桥的远期临床效果提供参考。方法同期制作CAD/CAM氧化锆烤瓷后牙固定桥(A组)、金合金烤瓷固定桥(B组)、镍铬合金烤瓷固定桥(C组)各20件。临床检查60件修复体边缘密合性,随访复查修复体的基牙继发龋和修复体的折裂、折断、脱落情况。结果 60件修复体平均随访3.0~3.5年。A、B组边缘密合性分别为100%和95%,明显好于C组(80%)(P<0.01)。继发龋的情况也是A组(0)、B组(0)好于C组(15%)(P<0.05)。随访期内,修复体折断数量A组为2例(10%),B组为1例(5%),C组无折断。崩瓷A组为1例(5%),B组为2例(10%),C组为4例(20%)。折断率和崩瓷率无统计学意义(P>0.05)。结论 CAD/CAM氧化锆陶瓷后牙固定桥边缘密合性好,能够达到传统金属固定桥的强度,满足临床应用要求。 Objective To evaluate and compare effects of all-ceramic bridges made by chari-side computer aided design and computer aided manufacture system with glod alloy and Ni-Cr alloy ceramic bridges in clinic.Methods Of a total of 60 bridges,20 were allceramic bridges made by chari-side computer aided design and computer aided manufacture system(Group A),20 were glod alloy ceramic bridges(Group B),and 20 were Ni-Cr alloy ceramic bridges(Group C),respectively.The marginal fitness and fracture of ceramic bridges were checked and caries or seond caries of all autments were also examined in clinic at 3.0-3.5 years after cementation.Results The marginal fitness of Group A was evaluated good in 100% of the bridges,which was better than the fitness of Group B(95%) and Group C(80%)(P0.01).Caries or seond caries of Group A were less than those of the other groups(P0.05).There was no defference of fracture of ceramic layer among all three groups.Conclusions The 3.0-3.5 year clinical study on all-ceramic bridges made by chari-side computer aided design and computer aided manufacture system presents good results in marginal fitness and fracture resisance,which indicates the all-ceramic bridges made by chari-side computer aided design and computer aided manufacture system can be used in clinic.
