FR-Ⅲ功能调节器矫治59例儿童前牙反软硬组织形态的变化研究

Study on anterior crossbite treated with FR-Ⅲ in 59 children

目的分析FR-Ⅲ型功能调节器对乳、替牙期患者面部软、硬组织的矫治变化的作用原理。方法采用FR-Ⅲ型矫治器对59例乳牙期及替牙期安氏Ⅲ类前牙反患者(4～9岁)进行治疗。分别在治疗开始(T0)和结束(T1)时拍摄头颅定位侧位片,SPSS13.0统计软件进行治疗前后的测量值配对t检验。结果与矫治前相比,ANB角、SNA角变大;下颌后下旋转,位置后移;全面高、下前面高、下面高/全面高比值增大;U1-NA角增大,L1-NB角减小;面型角、鼻唇角增大;上唇近中移动,下唇远中移动(P<0.05)。结论 FR-Ⅲ矫治器通过牙代偿和下颌骨的后下旋转及上齿槽的趋于丰满而有效纠正患者前牙反,面型改善,但颌骨的改变不显著。高角患者慎用。

Objective To study the role of FR-Ⅲ principle by the skeletal and soft tissue changes after treatment with FR-Ⅲ appliance.Methods 59 children of 4-9 year-old with class Ⅲ malocclusion were selected to treat exclusively with the FR-Ⅲ appliance for this study.Before and after the treatment,cephalometric radiographs were taken and compared in paired t-test with SPSS13.0.Results The ANB angle and SNA angle（P0.05） were increased.The mandible had a downward and backward rotation（P0.05）.ANS-Me and ANS-Me/N-Me（P0.05） were increased.U1-NA angle was increased,while L1-NB angle was decreased（P0.05）.There were increases in N＇-Sn-Pog＇ and Cm-Sn-UL angle（P0.05）.The upper lip moved mesially and the lower lip moved distally.Conclusions The anterior crossbite was corrected with clockwise rotation of mandible,lingual inclination of lower anterior and the labial inclination of upper anterior tended to be plump.The original concavity became better.However,there were no significant changes in jaws.Using FR-Ⅲ should be cautions with the children Angel Ⅲ malocclusion with hyperdivergen.
