SWI在弥漫性轴索损伤诊断中的价值 被引量:8

The value of susceptibility weighted imaging in diagnosis of diffuse axonal injury

摘要 目的:探讨磁敏感加权成像在急性期脑弥漫性轴索损伤(diffuse axonal injury,DAI)中的诊断价值。方法:回顾性分析23例经临床确诊DAI患者的影像学资料,所有患者均在常规MRI扫描基础上行液体衰减翻转恢复(FLAIR)序列及磁敏感加权成像(SWI)扫描,比较两者DAI病灶的检出率,分析信号特征。结果:联合常规序列和SWI共发现DAI病灶378个;其中SWI检出病灶341个,敏感度为90.2%,在磁敏感图(SWI image)表现为散在、大小不等且不规则形低信号影,边界清晰,在相位图(Phase image)测病灶较正常脑组织相位位移量明显大(P<0.05);在FLAIR检出病灶197个,敏感度为52.1%,表现为边界模糊的大小不等点片状高或稍高信号影,SWI对于DAI的检出率显著高于FLAIR(P<0.01)。结论:对于急性期DAI病灶的显示,SWI比常规序列敏感,具有更高的诊断价值。 Objective:To assess the value of susceptibility weihgted imaging (SWI) in diagnosis of diffuse axonal injury. Methods:The MRI data of 23 cases with DAI diagnosed by clinic were analyzed retrospectively. All the patients underwent MRI examination with FALIR sequence and SWI. The detection rates by SWI and FLAIR were compared and characteristics of signal were analyzed. Results:378 diffuse axonal injury lesions were detected by the combination of SWI and FLAIR totally. The sensitivity of SWI and FLAIR was 90.2%,52.1% individually. Conclusion:SWI is more sensitive than FLAIR in diagnosing DAI.
