摘要
目的评价多层螺旋CT血管成像(multi-slice computed tomography angiography,MSCTA)在诊断颅外颌面部动静脉畸形(arteriovenous malformation,AVM)中的价值。资料与方法对临床怀疑为颅外颌面部AVM的48例患者行MSCTA检查,扫描后通过容积再现(VR)、最大密度投影(MIP)处理以分析该病变的特殊表现。46例行数字减影血管造影(DSA)检查,17例行手术切除。结果 48例中,MSCTA显示畸形血管团者48例(100%);显示供血动脉者43例(89.6%);显示引流静脉者44例(91.7%)。13例(27.1%)软组织AVM合并有颅颌面骨异常(4例颌骨;9例其他颅面骨)。结论多数颅外颌面部AVM的表现特点能在MSCTA上清楚显示。作为一种无创检查方法,MSCTA能为临床诊断和治疗提供有益信息。
Objective The aim of this study was to display the value of multi-slice computed tomographic angiography(MSCTA) in the diagnosis of arteriovenous malformation(AVM) in extracranial maxilloface.Materials and Meth-ods Forty-eight patients with clinical suspicions of AVM in extracranial maxilloface performed MSCTA.The raw data of CT scan were reconstructed with volume rendering(VR) and maximum intensity projection(MIP).Forty-six of 48 patients performed digital substracton angiography(DSA),and 17 of 48 were resected.Results The malformed vascular was showed on MSCTA in all lesions(100%).Feeding arteries were displayed in 43 cases(89.6%) and draining veins were displayed in 44 cases(91.7%).Thirteen(27.1%) lesions had bone erosion.Conclusion Most of characteristic signs of AVM in the extracranial maxillofacial soft tissue can be clearly demonstrated on MSCTA.The AVM findings on MSCTA may offer valuable imformation for clinical diagnosis and treatment.
