探讨改良bacon术式联合全直肠系膜切除术在低位直肠癌保肛术中的应用

目的通过临床分析,探讨改良bacon术式联合全直肠系膜切除术在低位直肠癌保肛术中的应用效果。方法回顾分析我院2005年1月～2009年1月,28例低位直肠癌实施保肛手术的病例。结果全部病例均获随访,随访时间14～48个月。所有病例均行手术治疗,全组手术无死亡,切缘均经病理证实,除1例有肿瘤细胞浸润其余均无肿瘤细胞残留,无吻合口瘘,无切口感染,吻合口狭窄2例。1例于术后6个月局部复发,改Miles术,1例因其他疾病死亡。结论改良bacon术式联合全直肠系膜切除术(TME)应用在低位直肠癌保肛术,可减少术后复发,提高低位直肠保肛手术成功率。

Objective To explore the curative effects of Bacon with total mesorectum excision in anal sphincter operation for low rectal carcinoma.Methods The clinical data of 28 patients with low rectal cancer received anal preservative operation were analyzed respectively from Jan.2005 to Jan.2009.Results All the cases were followed up for at least 14 months to 48 months.28 patients were operated with no death,no anastomotic leakage and anastomotic infection.Only one case suffered remained cancer cells in cutting margin and two cases anastomotic narrow.One received Mile operation after 6 months because of local recurrence.The other died of other disease.Conclusion Improved Bacon operation and TME together could reduce postoperative recurrence and improve the successful rate of low rectal carcinoma anal sphincter preserving operation.
