钼铑双靶X线对乳腺导管内癌及微小浸润导管内癌的诊断价值

Value of Molybdenum and Rhodium Twin Target X-ray in Diagnosis of Breast Ductal Carcinoma in Situ and Intraductal Carcinoma with Micro-invasion

摘要 目的探讨钼铑双靶X线检查对乳腺导管内癌(DCIS)与微小浸润导管内癌(DCIS-MI)诊断及鉴别诊断价值。方法回顾性分析经病理证实的10例DCIS与6例DICS-MI患者的钼铑双钯X线片特征。结果 DCIS X线征象:①多发细小钙化5例,钙化范围局限;②局限性致密、结构不良7例,边缘部分清楚;③间接征象:伴有乳头凹陷1例,血运较对侧丰富1例,腋下见致密增大淋巴结影2例。DCIS-MI X线征象:①多发细小钙化4例,范围较广泛,钙化形态不规则,见模糊的钙化影;②局限性致密、结构不良2例,边缘不清;③间接征象:伴有乳头凹陷1例,2例皮肤有增厚,腋下见致密增大淋巴结影1例。结论肿块或局限性致密与多发细小钙化为DCIS及DCIS-MI最常见X线表现形式。对钙化的分析有利于术前判断病变是否浸润,有利于临床制定手术范围及手术方式的制定。 Objective To evaluate the value of molybdenum and rhodium twin target X-ray diagnosis and differential diagnosis of the breast ductal carcinoma in situ(DCIS) and intraductal carcinoma with micro-invasion(DCIS-MI).Methods Retrospective analysis X-ray imaging of 10 cases of DCIS patients and 6 cases of DCIS-MI patients,which confirmed by pathologically.Results X-ray of DCIS:①Five cases were featured micro-calcification,calcification confined;②Seven cases were featured with increased density or structural disorder.③Indirect signs:one case associated with inverted nipple,another rich blood supply than the contra lateral one case,and the axillary lymph node density increases in two cases.DCIS-MI X-ray:①Multiple small calcifications in four cases,the broader,irregular calcification,calcification blur;②Only showed local density or structural disorder in two cases,blurred edge;③Indirect signs:Inverted nipple in one case,two cases of skin thickness,and the axillary lymph node density increases in one case.Conclusion Dense mass or limitation and multiple small calcifications are DCIS and DCIS-MI X-ray the most common form.Analysis of the calcification disease help determine whether preoperative infiltration is beneficial to the scope of clinical surgery and surgical methods to determine the formulation.
