冠状切口加局部小切口治疗眶颧上颌骨复杂骨折

Treatment of maxillary complex fractures with coronal incision and located small incision

目的探讨冠状切口加局部小切口在眶-颧-上颌骨复杂骨折中的疗效。方法回顾性分析应用冠状切口加局部小切口治疗眶、颧、上颌骨复杂骨折患者35例,其中9例用冠状切口+睑缘下切口+口内切口;8例冠状切口+睑缘下切口;17例冠状切口+口内切口,含4例合并下颌骨骨折的用口内或颌下切口同期下颌骨骨折复位内固定。结果 35例均成功完成手术,术后X线检查,骨折愈合良好,34例为一期愈合,1例延期愈合,外形满意。出现1例暂时性额肌瘫痪,1例感面部皮肤轻度麻木。1例术口感染溢液,抗炎换药理疗后痊愈。结论冠状切口加局部小切口,疗效肯定、切口隐蔽,面部无瘢痕,且术野清晰,是上颌骨、眶、颧复合骨折切开内固定术的一种理想治疗选择。

Objective To investigate the efficacy of treating complex orbital zygomatic maxillary fracture with coronal incision combined with auxiliary small incisions.Methods 35 patients with complex orbital zygomatic maxillary fracture were retrospectively analyzed.Results All cases presented primary healing with satisfactory outline.X-ray showed well fracture healing.There were temporal paralysis of frontal muscle in one case and facial skin numbness in 1 case.Conclusion The coronal incision combined with auxiliary small incisions is ideal method for treating orbital zygomatic maxillary fractur.
