骨髓转移癌细胞形态学分析

Analysis of cell morphology of metastatic carcinoma of bone marrow

目的探讨骨髓转移癌细胞形态学特征及临床表现。方法对18例骨髓转移癌患者的临床表现及血象、骨髓象进行分析。结果血常规检查白细胞(WBC)降低者占61.0%;升高者占5.5%;血红蛋白(Hb)降低者占83.3%;血小板(PLT)降低者占66.7%;全血细胞均降低者占44.4%;外周血涂片出现幼稚红细胞者占77.8%;出现幼稚粒细胞者占55.6%;红细胞沉降率升高者占77.8%。18例骨髓片中均可找到数量不等单个出现或成团、成簇的转移癌细胞。结论骨髓细胞学检查是骨髓转移癌的简单而有效的诊断方法,有助于明确或未明确诊断的恶性肿瘤患者的治疗以及延缓病情发展。

Objective To investigate the cell morphology features and the clinical manifestations of metastatic carcinoma of bone marrow.Methods The clinical manifestations,hemogram and myelogram in 18 cases with metastatic carcinoma of bone marrow were collected and analyzed.Results The general blood examinations showed leukopenia in 61.0% of the patients,elevated leukocyte in 5.5%; reduced haemoglobin in 83.3%; thrombocytopenia in 66.7%; reduced whole blood cells in 44.4%; the presence of immature erythrocytes in peripheral blood smear in 77.8% and immature granulocyte in 55.6%; and elevated erythrocyte sedimentation rate in 77.8%.More or less metastatic carcinoma cells emerged as a single or formed group or cluster could be found evidently in the bone marrow smears in18 patients.Conclusion The bone marrow cytology examination is a simple and valuable method to diagnose metastatic carcinoma of bone marrow,which is helpful for the therapy of the patients clearly or unclearly diagnosed with malignant tumour and may slow down the progress of disease.
