稀释性自体输血在老年患者骨科大手术中的应用

Application of hemodilutional autotransfusion in major orthopedics in elderly patients

目的探讨稀释性自体输血(HAT)在老年患者骨科大手术中的有效性和可行性。方法回顾性分析60例≥60岁实施骨科大手术患者的临床资料,比较HAT组(n=30)和未实施自体输血(对照组,n=30)手术中异体血输血的比例和HAT组术前和术后第1天血红蛋白(Hb)?红细胞压积(Hct)和血小板(Plt)的变化。结果 HAT组和对照组输异体血比例分别为10%和60%,有显著性差异(P<0.05);HAT组术前和术后第1天Hb、Hct和Plt比较,差异均无统计学意义(P>0.05)。结论 HAT能有效减少老年患者骨科大手术中输异体血的概率。

Objective To discuss the validity and feasibility of hemodilutional autotransfusion （HAT）in major orthopedics in elderly patients.Methods The clinical data of 60 patients aged 60 years and over who received a major orthopedics were analyzed retrospectively,and the xenogenous blood transfusion rate between HAT group （30 cases） and control group （30 cases） was compared.The blood routine index,including Hb,Hct and Plt of HAT group were detected and compared before operation and the first day after the operation.Results The xenogenous blood transfusion rate of HAT group and control group was 10% and 60% respectively（P0.05）.There was no significant difference in Hb,Hct and Plt before the operation and the first day after the operation in HAT group（P0.05）.Conclusions HAT can reduce the chances of xenogenous blood transfusion in major orthopedics in elderly patients.
