尿视黄醇结合蛋白与窒息新生儿肾损伤的关系

摘要 目的:探讨尿视黄醇结合蛋白(RBP)在窒息新生儿肾功能损伤的诊断价值。方法:选取轻度窒息新生儿(n=56)和重度窒息新生儿(n=34)作为试验组,另选正常新生儿(n=100)作为对照组,分别检测出生后首次和24小时左右尿RBP/Cr,并同时测血尿素(Urea)和血肌酐(Cr),将检测结果进行比较分析,以评估尿RBP的应用价值。结果:重度窒息组两次尿RBP/Cr均值都高于轻度窒息组,且明显高于对照组,差异有统计学意义(P<0.05),但两组间血Urea和血Cr均值差异无统计学意义(P>0.05)。研究组生后24小时内尿RBP/Cr、血U-rea、血Cr出现异常的比率分别是90.0%(81/90)、27.8%(25/90)、33.3%(30/90);尿RBP/Cr异常率明显高于后两项指标,差异有统计学意义(P<0.01);血Urea与血Cr之间的异常率比较,差异无统计学意义(P>0.05)。结论:尿RPP/Cr可作为早期监测窒息新生儿肾功能损害的敏感指标,且比血Urea和血Cr敏感,有便捷、经济、取材容易和可连续监测的优点,值得临床推广。 Objective:To discuss the practical value of RBP to the renal injury after neonatal asphyxia.Methods:to choose light-asphyxia neonatal(n=56) and heavy-asphyxia neonatal(n=34) as the experimental groups,and another normal neonatal(n=100) as the comparative group,later take their first and about 24-hour-later urine to test their urine RBP/Cr.At the same time Urea and Cr should also be tested to compare the test result to evaluate the practical value of RBP.Results: The group with heavy-asphyxia has higher urinal RBP/Cr compared with the group with light-asphyxia both in their first and 24-hour-later urine tests,and apparently higher than the comparative group.This difference has statistics significance(P〈0.05),however,the average divergence between the blood Urea and Blood Cr of this two group has no statistics significance(P〉0.05),In the 24 hours later after being born urine test of the experimental group,the ratio of appearing abnormal phenomena in RBP/Cr,blood Urea,blood Cr is respectively 90.0%(81/90)、27.8%(25/90)、33.3%(30/90).The rate of Urinal RBP/Cr is abnormally higher than the latter two.And This difference has statistics significance(P〈0.01);The unusual rate of difference between blood Urea and Blood Cr has no statistics significance(P〉0.05).Conclusions: Urine RPP/ Cr which is more sensitive,available and successive compared with that of blood Urea and blood Cr can be a sensitive index to measure the renal injury after neonatal asphyxia,and is worth the clinical application.
