沐舒坦联合纳洛酮治疗胎粪吸入综合征所致呼吸困难74例疗效观察

摘要 目的观察沐舒坦、纳洛酮联合治疗胎粪吸入综合征所致呼吸困难的疗效。方法自2003年1月至2007年10月在我科就诊的胎粪吸入综合征所致呼吸困难74例,随机分为治疗组40例和对照组34例。两组均给予常规治疗,治疗组在常规治疗基础上加用沐舒坦、纳洛酮,其中沐舒坦每次7.5mg/kg,q6h1次;纳洛酮针首次负荷量0.1mg/kg静推,1小时后按0.01mg/(kgh)的速度经微量输液泵持续静脉注射,每日总量不超过0.4mg/kg。结果在治疗结束时,治疗组显效27例(67.5%),有效11例(27.5%),总有效率为95.0%(38/40)。对照组显效15例(44.1%),有效10例(29.4%).总有效率为73.5%(25/34)。两组显效率比较差异有显著性意义(X^2=4.09,P〈0.05),总有效率比较差异有非常显著性意义(X^2=6.69,P〈0.01)。结论沐舒坦联合纳洛酮治疗胎粪吸入综合征所致呼吸困难疗效显著值得临床进一步研究。 Objective To evaluation the effect of mucosolvan in combination with naloxone on meconium aspiration syndrome (MAS). Methods Seventy-four patients with MAS in our department from January 2005 to Januray 2006, were divided randomly into control grop (n=34) and the treatmets group (n=40) .All patients were treated by conventional therapy, and treatmets group were given combined with mucosolvan and naloxone , and mucosolvan (7.5mg/kg) was given every six hour ; naloxone (0.1 mg/kg) was injected into the blood-stream for the first time, after an hour was given at the stable rate of 0.01 mg/kg an hour by microtransfuseon system (total≤〈0.4mg/kg·d) .Results At the end of the course, 27 newborns in the treatmets group had remarkable effect with the rate of 67.5% (27/40), 11 had effect with the rate of 27.5% (11/40), the total rate was 95.0% (38/40) . And 15 in the control group had remarkable effect with the rate of 44.1% (15/34), 10 had effect with the rate of 29.4% (10/34), the total rate was 73.5% (25/34) .There was significant difference between the two grouops in the remarkable rate, and very significant difference between the two groups in the remarkable.Conclusion There are a significant curative effect of the patients with MAS after given mucosolvan and naloxone , and worth further investigating.
