溶血磷脂酸、神经元特异性烯醇化酶及纤维蛋白原在进展性脑梗死中的变化 被引量:5

摘要 目的通过检测溶血磷脂酸(LPA)、神经特异性烯醇化酶(NSE)及纤维蛋白原水平,探讨其在进展性脑梗死发病机制的作用及临床意义。方法 205例急性脑梗死患者,根据病程演变,分为进展组(64例)和非进展组(141例),测定各组LPA、NSE及纤维蛋白原含量,并与正常对照组(50例),进行对比。结果 LPA、NSE、纤维蛋白原含量分别为进展组:(8.62±2.02)μmol/L,(13.56±8.67)μmol/L,(4.62±1.13)g/L;非进展组:(4.93±1.03)μmol/L,(7.49±3.25)μmol/L,(3.62±0.88)g/L;对照组:(2.55±1.13)μmol/L,(2.85±1.33)μmol/L,(3.57±0.86)g/L,进展组LPA、NSE均高于非进展组及对照组,组间比较差异有显著性,纤维蛋白原进展组高于非进展组及对照组,结果有显著性差异(P<0.05),非进展组与对照组之间无显著性差异(P>0.05)。结论外周血LPA、NSE及纤维蛋白原在进展性脑梗死患者明显增高,可作为进展性脑梗死的预警因子,对进展性脑梗死患者的早期诊治有重要参考价值。 Objective To explore the clinical meaning of lysophosphatidic acid(LPA),Neuron specific enolase(NSE) and fibrinogen in progressive cerebral infarction by testing their levels.Methods Two hundred and five patients were classified in two groups according to the progression of symptoms: progressive cerebral infarction group(n=64) and non-progressive cerebral infarction group(n=141).LPA,NSE and fibrinogen were tested in each group and compared with those in control group(n=50).Result The levels of LPA,NSE and fibrinogen in progressive cerebral infarction group were(8.62±2.02) μmol/L,(13.56±8.67) μmol/L and(4.62±1.13)g/L;in non-progressive cerebral infarction group were(4.93±1.03) μmol/L,(7.49±3.25) μmol/L and(3.62±0.88) g/L;in control group were(2.55±1.13) μmol/L,(2.85±1.33) μmol/L and(3.57±0.86) g/L.LPA and NSE in progressive cerebral infarction group were higher than those in the other two groups.They were significantly different among the three groups.Fibrinogen in progressive cerebral infarction group was higher than that in the other two groups.But they were not significantly different between non-progressive cerebral infarction group and control group.Conclusion LPA,NSE and fibrinogen of progressive cerebral infarction were significantly higher than those in non-progressive cerebral infarction group and control group.They could be prognostic factors and might play an important role in clinical diagnosis and treatment.
