经下腹部正中切口后入路在腹股沟疝急诊手术中的应用

Posterior approach to emergency inguinal hernia via hypogastric midline incision

目的探讨腹股沟疝急诊手术中经下腹正中切口后入路的优势。方法选取我院2001年1月至2008年12月腹股沟疝急诊病例47例,年龄17-80岁,分为两组,A组为经下腹部正中切口后入路组21例,B组为前入路组26例,对两种手术方法进行回顾性分析。结果经下腹部正中切口后入路组相比前入路组解除嵌顿速度较快,放置补片进行全耻骨肌孔修补,更符合修补原理,并可以经同一切口对腹腔进行较全面探查。结论经正中切口腹膜前入路在腹股沟疝急诊手术中较经腹股沟切口实用。

Objective To investigate the advantage of the posterior approach to emergency inguinal hernia via hypogastric midline incision.Methods 53 cases with the emergency inguinal hernia were selected from January of 2001 to December of 2008,aged 17 to 80,and were divided into two groups.A group had 21 cases,which operations were posterior approach via hypogastric midline incision,and the B group had 25 cases,which operations were via anterior approach.Results Posterior approach via hypogastric midline incision was faster than anterior approach in relieving hernia,and placing the plug in the preperitoneal space for total repair of the myopectineal orifice,which was more reasonable,if necessary,via hypogastric midline incision could check all-around abdominal cavity.Conclusion Posterior approach to emergency inguinal hernia via hypogastric midline incision was more functional than anterior approach.
