摘要 目的探讨高频超声在桡神经病变中的临床应用价值。方法应用高频超声检查24例桡神经病变患者,将桡神经病变分为神经卡压、断裂、肿瘤。观察各类病变的声像图特征,并以术中所见为标准。结果 14例卡压性损伤声像图表现为卡压处神经变细,束状回声不清晰,远端水肿增粗;5例神经断裂声像图表现为桡神经连续性部分或全部中断,断端神经束粗大,回声减低。2例神经肿瘤表现为低回声肿物,边界清晰、椭圆形,其两端与神经干相连。3例创伤性神经瘤表现为与神经游离端相连的低回声膨大,内部回声较均匀。超声诊断符合率91%。结论高频超声可为桡神经病变的形态学观察提供新的影像检查方法。 Objective To assess the value of the high-frequency ultrasound for diagnosing radial nerve lesions.Methods 24 patients with radial nerve lesions were detected with high-frequency ultrasound.The radial nerve leisions were classified into nerve compression,nerve disruption and nerve tumor.The ultrasonic features of each type of lesions were observed and compared with the intraoperative findings.Results 14 patients with compression appeared as thinner on nerve entrapment,nerve bundle echo was not clear,thickened distal edema.5 patients with radial nerve disruption showed the nerve continuity of some or all of interruption,the ends nerve bundles thick,echo reduction.2 patients with nerve tumors showed hypoechoic masses,clear boundary,oval,and connected with two ends neural stem.3 patients with traumatic neuroma showed hypoechoic enlargement connected with the nerve distal and the internal echo was uniform.The ultrasonic accuracy was 93%.Conclusion High-frequency uhrasonography provides good quality brachial plexus ultrasound images and shows great potential in noninvasive diagnosis of radial nerve lesions.
